Immunocompromised Infection with Behçet Disease

Immunocompromised Infection with Behà §et Disease Abstract Behà §et disease is a relapsing multiorgan inflammatory disorder characterised by mucocutaneous, opthalmic, neurological, vascular and gastrointestinal involvement. We report an intriging case with rare manifestations of myositis and myocarditis, and the first reported occurrence of an immunocompromised-associated infection (polymicrobial necrotising fasciitis) without immunosuppresive therapy use in this presumably hyperimmune disorder. Lessons from practice: 1. Behà §et Disease is a rare autoimmune condition with significant geographical variation in its distribution and is most prevalent in people of Turkish background. 2. Behà §et disease is a systemic disease with predominant symptoms of oral ulcerations, genital ulcerations and uveitis. Other systemic involvement include neurological, gastrointestination, rheumatic, dermatological manifestation. Less common but well described manifestation include muscle and cardiac involvement. 3. The unexplained manifestation of Type 1 necrotising fascitis in this patient suggests that immune dysregulation in Behà §et disease could contribute to primary immunosuppression. It is important to maintain high viligance for risk of infection in Behà §et disease. Clinical Record A 40-year-old woman presented to hospital for investigation of progressive malaise, anorexia and generalised proximal myalgia over 6 days with an elevated serum creatine kinase [CK] of 3550 U/L (reference range [RR] < 145). She is of Spanish and Turkish ethnic background. Her medical history included transfusion-related Hepatitis C Virus (HCV) infection, lower limb deep vein thrombosis with Factor V Leiden mutation and depression. She denied any infective symptoms, recent trauma or stenuous exercise, rash or athralgias. Physical examation was unremarkable except for proximal generalised myalgia without weakness. Laboratory investigations initially revealed a normal erythrocyte sedimentation rate and C-reactive protein (CRP). Over the next 72 hours, she developed an acute bilateral polyathropathy of the wrist and metacarpalphalangeal joints. Multiple cutaneous manifestations were observed including (1) a rapidly evolving erythema nodosum-like rash over the legs which later progressed into a purpuric non-blanching rash with fluctuant subcutaneous oedema, (2) multiple oral aptheous ulcers and (3) perioral acneiform papular nodules. Further laboratory studies showed mildy raised inflammatory markers and a normalising CK of 428 U/L. The results of investigations for differential diagnoses were unremarkable. (Table 1) Multiple skin biopsies showed a neutrophilic-dermatosis like reaction. On day 6, she developed a mildly tender natal cleft excoriation which progressively became pustular over 2 days with a dramatic increase in CRP >380mg/L and worsening renal function. She remained systemically well. A wound swab grew methicillin-sensitive staphylococcus aureus. Oral flucloxacillin and metronidazole were commenced. MRI showed enhanced T2 signals in the presacral fascia correlating to the sacral wound. A biopsy of the natal cleft wound showed similar findings to previous skin biopsies. Simultaneously, a pathergy-like reaction was noted with previous intravenous-cannula and biopsy sites becoming pustular. At this point, further clinical history revealed a 5 year history of recurrent oral and gential ulcerations up to 4 episodes a year. A clinical diagnosis of Behà §et disease was made. Subsequent HLA-B51 testing and ophthalmological examination were unremarkable. On day 10, she deteriorated acutely becoming hypotensive and hypoxic. Computer tomography (CT) pulmonary angiography revealed bilateral pneumonia. Serum white cell count and CK increased substantially from 11.0 x109/L to 37.2 x109/L (RR 4.0 – 11.0) and 233 U/L to 3715 U/L respectively in 4 days. She was intubated, commenced on broad-spectrum antibiotics with systemic glucocoticoid and inotropic support. A further CT of the pelvis revealed numerous sacral subcutaneous gas locules and fluid. Multiple debridement procedures achieved drainage of large amount of pus and excision of the necrotic tissue. Sacral necrotising myofasciits was confirmed on a frozen section specimen showing extensive necrosis within subcutis, fascia and skeletal muscle with numerous mixed bacteria. Employees are the part in the company that understand and know what the consumer expected from the service or product. So, employee should can predict what product or service which can give the customers satisfaction. Conducting the training program is one of the opportunities that the company takes. Training program is one of the best choices to improve the employees’ job performance and increase incomes. Training process is not easy. There must be some steps to do for reaching the goal. Every forward looking company realized that introduced training program to old and new employee is needed. Giving the training to the employee is the company responsible. The superior in the company sure have to involve in the training process to lead the employees. It needs a lot of preparation to determine which training programs have to be prepared and it is a problem that the company has to face. PT. CALISPO MULTI UTAMA face a problem which is they face obstacles in increasing the employees work performance. The poor of training program is very affecting the employees’ job performance. From the problem above, the writer would like to formulate her analysis of PT. CALISPO MULTI UTAMA throughout her studies with topic â€Å"the effective training method to improve the employees’ work performance â€Å". C. Hypothesis Hypothesis is the principle in the solving the problem based on the research which has not yet proved to be correct. Based on the research that I have done at PT. CALISPO MULTI UTAMA, which is â€Å"the more the company conducting the training, the more employees’ job performance will increase†. So, by conducting the training, it will show the changes in the result of employee job performance. D. The Method of Research Here I gather the information by using three method of research. First, the information from electronic media that is by internet research. Browsing the internet can give a lot of important information. From the internet, there are also a lot of sample of companies’ problems and the solutions. So from here, we can gain more information to completing this thesis. Second, the information from non-electronic media that is books from library and bookstores. There are a lot of book of management and other article or magazine which can tell more about the topic of this thesis. Third, is by field research. In this method, it is needed to do interview, giving questioners and gather primary data from the company. The objective of this research is to know haw far is the training program has been conducted in the company, how the employees’ performance been done. Field research is the most importance because we directly face the people and directly get the information. E. The Overview of Contents Chapter I:Introduction This chapter consists of the background of selecting the title, problem identification, hypothesis, the method of research, and the overview of contents. Chapter II:Theoretical Review This chapter consists of human resources management, motivation, job evaluation, transfer and training and development. Chapter III:The Company Profile This chapter consist a brief history of the company, the organization structure of the company, job description and implementing training system. Chapter IV:Analysis and Evaluation This chapter consists of the method of training in improving the employees’ job performance, the purpose of training, training design and measure of success by the writer about PT. CALISPO MULTI UTAMA. Chapter V:Conclusion and Suggestion This chapter consists of the conclusion and suggestion that make by the writer about PT. CALISPO MULTI UTAMA to help the company to solve the problem to reach the company goal.

Designing A Multimodal Composition About My Writing Essay

For project three, the objective was to create a multimodal composition about my writing process. Throughout the poster, I talked about how my writing has changed as well as how it has stayed the same since I began English 101. I also spoke on the struggles I went through with writing in high school. Through my poster, I was able to convey how a deeper understanding of the writing process in high school can help mediate these struggles and better prepare one for their intro Writing class in college. Many of my writing traits have changed since I have been in English 101. While being in English this semester my writing has continued to improve greatly. This was mostly due to what I have changed since high school. One thing that I have changed is the way I plot out my essays. The way I was going about it before did not work for me because of the format that my previous teachers required. Now, after being in college, I can freely set up my essays in a way that I am comfortable with and can understand if I need to look back at them later. What Happens If You Mix an Acid and a Base

Mixing an acid with a base is a common chemical reaction. Here is a look at what happens and the products resulting from the mixture. Understanding the Acid-Base  Chemical Reaction First, it helps to understand what acids and bases are. Acids are chemicals with a pH less than 7 that can donate a proton or H ion in a reaction. Bases have a pH greater than 7 and can accept a proton or produce an OH- ion in a reaction. If you mix equal amounts of a strong acid and a strong base, the two chemicals essentially cancel each other out and produce a salt and water. Mixing equal amounts of a strong acid with a strong base also produces a neutral pH (pH 7) solution.  This is called a neutralization reaction and looks like this: HA BOH → BA H2O heat An example would be the reaction between the strong acid HCl (hydrochloric acid) with the strong base NaOH (sodium hydroxide): HCl NaOH  Ã¢â€ â€™ NaCl H2O heat The salt that is produced is table salt or sodium chloride. Now, if you had more acid than base in this reaction, not all of the acid would react, so the result would be salt, water, and leftover acid, so the solution would still be acidic (pH 7 ). If you had more base than acid, there would be leftover base and the final solution would be basic (pH 7). A similar outcome occurs when one or both of the reactants are weak. A weak acid or weak base doesnt fully break apart (dissociate) in water, so there may be leftover reactants at the end of the reaction, influencing the pH. Also, water may not be formed because most weak bases are not hydroxides (no OH- available to form water). Gases and Salts Sometimes gases are produced. For example, when you mix baking soda (a weak base) with vinegar (a weak acid), you get carbon dioxide. Other gases are flammable, depending on the reactants, and sometimes these gases are flammable, so you should use care when mixing acids and bases, especially if their identity is unknown. Some salts remain in solution as ions. For example, in water, the reaction between hydrochloric acid and sodium hydroxide really looks like a bunch of ions in aqueous solution: H(aq) Cl-(aq) Na(aq) OH-(aq) → Na(aq) Cl-(aq) H2O Other salts are not soluble in water, so they form a solid precipitant.
